Regsitered Nurse, Health Risk Assessment Advisor
A registered nurse is responsible for providing nursing services to individuals. Nursing actions are directed toward the goals of prevention, assessment, risk reduction, and health status improvement for individuals and families. Nursing activities include, but are not limited to, systematic analysis of health data, care plan development, health education and advocacy.
  1. Responsible for inbound and outbound calls to conduct a comprehensive health risk assessment to identify all active and chronic disease conditions as well as determine all physical, mental and social needs.
  2. Knowledge of Prochaska and DiClemente's Stages of Change Model
  3. Maintains accurate and detailed records in systems.
  4. Provides health behavior education, emphasizing importance of consistent monitoring of health needs, and recommending resources.
  5. Acts in a professional manner reflecting the mission of the company, and demonstrates compliance with all policies, procedures and federal/state regulations.
  6. Consults and coordinates with supervisor to assess, plan and implement processes.
  7. Protects operations by keeping information confidential.
  8. Interacts with team to provide expertise and assistance in resolving client issues.
  9. Provides excellent customer service to staff, employers, and clients.
  10. Works schedule as established by supervisor to accommodate business hours and project services.
  11. Make recommendations to project supervisor to support the continuing improvement of the project.
  12. Performs other duties as may be assigned by management.
    Education Required: Bachelor's degree from an accredited college and valid nursing license.

    Education preferred: Bachelor's degree with at least 1 year of experience preferred.

    Skills / Knowledge Required: Licensed as a Registered Nurse recognized by the State of Michigan. Knowledge of nursing care in accordance with regulations and practices established.

    Skills / Knowledge Preferred: Medicaid-specific utilization and quality review experience preferred knowledge of state and federal managed care programs; Bilingual, preferably Spanish or Arabic speaking; and knowledge of demographic to be served.

    Experience Preferred: One to two years of experience in a related area.
    Abilities: Ability to maintain confidentiality at all times; ability to work as a team as well as independently; ability to multi-task; ability to promote healthy behavior and assist with health maintenance; ability to maintain a patient and a professional demeanor in high-stress, fast-paced settings; ability to communicate effectively (verbal and written); high ethical integrity; excellent attention to detail; ability to adapt to different situations and demographics of people; and ability to explain diagnosis, treatment options, and other essential information.
